Transcript Requests
Click here to request and pay for transcripts on line. You will be directed to a secure payment site. Please make sure to give us the student name, maiden name if applicable and names and addresses of colleges requesting the transcript. Each transcript is $5.00.
If you prefer, you may also download and print the Transcript Request Form, complete and return along with a check for $5.00 for each transcript requested to:

Transcripts will be sent out within two days after receipt of the request form and the $5.00 fee per transcript.
